How to Apply for MAHPC Registration as a Physiotherapist in Malaysia
A current, source-linked guide to MAHPC registration, recognised physiotherapy qualifications, MHPS documents, fees, practising certificates and the separate route for non-Malaysians.
Editorial illustration with fictional sample details. It is not an MAHPC-issued document, portal screen or government endorsement.
1. Identify the correct route first
Malaysian citizen, local qualification
Match the exact institution and qualification against the MAHPC recognised list before applying through MHPS.[MAHPC qualification list]
Malaysian citizen, overseas qualification
Section 16 still requires a qualification recognised by the Council. If the exact qualification is absent, obtain current assessment and MHPS-selection instructions directly from MAHPC; do not choose solely from the 2022 manual screen.[Act 774, section 16][Official MAHPC page]
Non-Malaysian
This is not the ordinary citizen-registration route. Section 24 provides a Temporary Practising Certificate for non-Malaysians who meet specified statutory categories, and MAHPC publishes supporting documents for expatriate applicants.[Act 774, section 24][Expatriate supporting documents]
2. Check eligibility and the physiotherapy division
Under section 16, an ordinary registration applicant must be a Malaysian citizen, hold a qualification recognised by the Council, fulfil the Council’s prerequisites and not have a conviction described in that section.[Act 774, section 16]
The masterlist separates Physiotherapist Division 1, generally the listed bachelor-level physiotherapy qualifications, from Division 2, the listed diploma qualifications. The physiotherapy section states that it was updated on 28 November 2025.[MAHPC List of Recognised Qualifications]
Match the institution, programme title and qualification level exactly; a similar-sounding title is not automatic proof of recognition.[MAHPC List of Recognised Qualifications]
3. The transition was extended, but do not guess the portal choice
The 2023 Registration Guideline recorded the original section 46 transitional period as 1 July 2022 to 30 June 2025. An official MAHPC media release dated 23 June 2025 subsequently said the registration transition was extended by 18 months to 31 December 2026.[2023 Registration Guideline][MAHPC media release, 23 June 2025]
The media release directs practitioners to submit registration by 31 December 2026 and obtain a valid Practising Certificate within the extended period. It does not explain which MHPS dropdown applies to every qualification type, so do not apply through the Section 46 transitional route solely because the 2022 manual displays it. If an institution or qualification is absent, obtain current instructions from MAHPC.[MAHPC media release, 23 June 2025][2022 MHPS Registration Manual][Official MAHPC page]
4. Create an MHPS account
- Open the official Malaysian Healthcare Practitioners’ System (MHPS).
- Register using your name, NRIC, email, password and captcha.
- Activate the account through email, then log in and complete your profile, photograph and address.
- Add the profession, institution, qualification, qualification date and supporting documents.
These core fields and steps appear in the registration manual still linked by the official MAHPC page; where the current portal screen differs, follow the live portal labels and current MAHPC instructions.[MHPS Registration Manual][Official MAHPC page]

5. Prepare the documents
- Academic qualification and transcript. The guideline lists the qualification copy, transcript where available, passport photograph and fee.[2023 Registration Guideline]
- Certified copies. Uploaded documents must be certified by a government officer in the management and professional group or a Commissioner for Oaths; the MHPS manual also requires certified true copies before upload.[2023 Registration Guideline][MHPS Registration Manual]
- Translations. Documents not in Malay or English must be translated by an accepted translator listed in the guideline, including a Malaysian Court Translator, Notary Public, ITBM or the Malaysian Translators Association.[2023 Registration Guideline]
- Practice Verification Form. The 2023 guideline lists it among the application documents, while the 2022 MHPS manual says it is only required for current practice. Because these instructions are not fully aligned, prepare it with the relevant evidence if you currently practise and confirm the current requirement with MAHPC in other situations.[2023 Registration Guideline][MHPS Registration Manual]
- Letter of Good Standing. The guideline lists it where the applicant has practised outside Malaysia; obtain it from the relevant professional or regulatory body for the place of practice.[2023 Registration Guideline]

New graduates who have not received the scroll
A MAHPC circular dated 6 May 2025 says new graduates who completed their studies but have not received the official scroll may submit a university-issued confirmation of completion and a certified full academic transcript. The circular remains linked from the official MAHPC page and states no expiry date for this arrangement; recheck the official page when applying.[MAHPC circular, 6 May 2025][Official MAHPC page]
6. Submit and pay at the correct stages
The official flow shows an RM50 registration application fee, Secretariat document review and Council consideration; after approval, RM100 is charged for issuing the Certificate of Registration.[2023 Registration Guideline, fee table and process flow][Official MAHPC registration and fee brochure]
The MHPS manual shows the declaration, preview, submission, FPX payment and receipt steps; after approval, the applicant logs in again for the certificate-issuance payment and downloads the Certificate of Registration.[MHPS Registration Manual]
MAHPC may request additional documents or an assessment before deciding. This guide does not promise a processing time because the actual timeline depends on document completeness, Secretariat review and Council determination.[2023 Registration Guideline]
7. After registration: apply for a Practising Certificate
Registration approval results in a Certificate of Registration and an entry in the Register; it does not by itself complete the authority to begin practice. A registered practitioner who intends to practise must separately apply for a Practising Certificate.[Act 774, sections 17 and 22]
The application requires the principal place of practice and all other practice addresses. An issued Practising Certificate is valid for two years, and an address change must be notified to the Registrar within 30 days.[Act 774, section 22]
The fee table lists RM50 for the Practising Certificate application and RM200 for issuance. The 2025 manual shows applicants reviewing or adding practice addresses, making the declaration and payment, then returning after approval for the issuance payment and certificate download.[MAHPC Fee Table][2025 Practising Certificate Manual]

8. Renewal and practice addresses
The guideline says a renewal application should be made not later than the first day of November in the year the Practising Certificate expires.[2023 Registration Guideline, renewal]
Do not wait for expiry before checking the MHPS profile, practice addresses and payment status. Act 774 also says a practitioner who does not apply for renewal for a continuous lapse of six years may be removed from the Register.[Act 774, sections 18 and 23]
